Chorley & South Ribble Events Performer Application Form 2026.

Thank you for your interest in performing at our events in Chorley and South Ribble in 2026. We are trying to build a database of quality performers that we can call upon to perform at our council run events.

This application form will provide us all of the relevant information we need from you as a performer, to allow us to book you for performances at our events. As part of this application process we will require a Public Liability Insurance certificate (minimum £5,000,000 cover) and a Risk Assessment to cover the basic risks that may occur whilst you perform at our events.

We require this information in advance to allow the application process for each event to become streamlined.

This information will be used by the council Event Officers who will be in charge of managing the events.  

This is an application form, completion of this form does not guarentee you a performance at our events. 

Please note that this performer application form does not include the Chorley Live, Penwortham Live and Longton Live events. To perform at these events please contact info@thecreativenetwork.org.uk
